Please read this contract carefully as it is a legally binding document with Animal Welfare League of Montgomery County. Initial all paragraphs and complete the last page. All fields are mandatory. By signing, the adopter agrees that he/she has read and understands the following terms and conditions of the adoption.

Terms & Conditions*
- I agree to care for this cat for the remainder of its life. This includes giving the cat proper food, water, shelter, exercise and kind treatment.
- I will take this cat to a licensed veterinarian for annual exams and routine immunizations, and will provide immediate veterinary care in the event that this cat becomes ill or is injured.
- I am obtaining this cat for myself. I will not sell, abandon, or give this cat to another party as a gift.
- I will keep this cat as a domesticated house pet. I will keep this cat indoors.
- I agree to comply with all state and local laws pertaining to guardianship/ownership of this cat.
- I certify that I have never been subject to legal action for cruelty or neglect of animals. I certify that I have never owned an animal that was confiscated for violations of state or local animal control regulations or animal adoption agreements.
- This cat will not be declawed or have the tendons attached to its claws cut.
- If I am unable to keep this cat for some reason I will notify AWLMC first.
- I agree that AWLMC shall have the right to immediate possession of this cat if, in the judgment of AWLMC, this cat is receiving improper care, or terms and conditions of this contract have not been met. I agree to pay any court costs and attorney fees incurred by AWLMC in enforcing this contract.
